School Entry Requirements

Sports Physical:  Any student who will be in grades 7-12 for the 2008-09 school year, and wishes to participate in extracurricular athletics must have a current sports physical done after May 1, 2008 as directed by the Kansas State High School Activities Association.

Immunization record:  please bring a copy with you when enrolling, if you are new to the district.

    Students entering grades 6-12 for the 2008-09 school year

        - DTaP (5)

        - Polio (4)

        - MMR (2)

        - Tetanus booster--10 years after the last DTaP

            - (usually 8th -10th grade)

   

Students in Booster Kindergarten and K-5 for the 2008-09 school year:

        - DTaP (5)

        - Polio (4)

        - MMR (2)

        - Hepatitis B (3)

        - Varicella (chicken pox)  or proof of disease

           

Administer Medicine: All medicines distributed at an Abilene School must have a Request to Administer Medicine form on file.

School Entry Physical: Any student between the ages of 5-8 who is enrolling in a Kansas school for the first time must also present a completed physical examination that has been done in the previous 12 month period.
